USPS VERA PSHB Medicare Part B: Health Insurance Rules for Postal Early Retirees
PSHB Replaced FEHB for Postal Workers
Since January 1, 2025, every career postal employee and postal annuitant is covered under the Postal Service Health Benefits Program. PSHB is a separate insurance framework managed by OPM alongside — but distinct from — the Federal Employees Health Benefits program that covers the rest of the executive branch.
If you're evaluating a VERA offer, the first question is whether you can carry health coverage into retirement. The answer depends on one rule and one waiver.
The Five-Year Enrollment Rule and the VERA Waiver
Under normal retirement, you must have been enrolled in FEHB or PSHB for the five years of service immediately before your retirement date or, if you have less than five years of service, for all service since your first opportunity to enroll. Employees who've been with the Postal Service for 20-plus years rarely have gaps. But the rule still trips up anyone who dropped coverage briefly — to switch to a spouse's plan, for example, or during a break in service.
Here's what matters for a VERA: OPM may include a pre-approved waiver of the five-year enrollment requirement in its authorization. The waiver can remove that five-year test, but its terms are specific to the authorization. Confirm in writing with HRSSC Greensboro that a waiver applies to your VERA and ask what conditions it includes.
The catch: the waiver is not automatic by statute. It depends on OPM including it in the specific VERA authorization. Before signing any retirement paperwork, confirm in writing with HRSSC Greensboro that a pre-approved PSHB waiver is active for your VERA round. Don't rely on workroom floor assurance — get it documented.
Medicare Part B Enrollment at 65
The Postal Service Reform Act of 2022 added a requirement for PSHB annuitants. When a PSHB-covered postal annuitant becomes eligible for Medicare Part A, they generally must also enroll in Medicare Part B to keep their PSHB coverage, subject to statutory exceptions.
Medicare Part B comes with a monthly premium. In 2026, the standard Part B premium is $202.90 per month (higher for modified adjusted gross income above $109,000 for an individual / $218,000 for a joint return). PSHB plans are designed to coordinate with Medicare, so your total out-of-pocket may actually decrease once Part B kicks in, but the premium itself is a new cost that pre-2025 postal retirees under FEHB did not face.
If you accept a VERA at age 50 or 52, this rule doesn't hit for another 13-15 years. But it's part of your long-term planning.

Limited Exceptions to the Part B Mandate
Not everyone must enroll. OPM lists five exceptions: enrollment in VA healthcare; eligibility for Indian Health Service care; residence outside the United States; specified grandfathering for a postal annuitant who was not both entitled to Medicare Part A and enrolled in Part B on January 1, 2025, or for a USPS employee who was at least 64 on that date; and certain Medicare Part A-only cases under Social Security Act sections 1818 or 1818A. TRICARE coverage by itself is not on OPM's list. Check your status with OPM before declining Part B.
What This Means for Your VERA Decision
PSHB coverage under a VERA works in your favor if you meet the five-year enrollment rule or have an applicable waiver. Your PSHB plan can continue into retirement with the government premium subsidy, and you pick a new plan during Open Season each year just like you do now.
The Part B mandate adds a cost your regular FEHB-covered colleagues in other agencies won't face. For most affected annuitants, it applies whether they retire under VERA or under regular optional retirement — it's a PSHB rule, not a VERA-specific penalty.
